MEDICAID Funded Mental Health and Mental Retardation  Services FAIR HEARINGS

If you are currently receiving Medicaid benefits and:

  • are requesting mental health or mental retardation case management or mental health rehabilitative services and those services have been denied by Andrews Center; or
  • have been receiving these services from Andrews Center and they are terminated or reduced,

you have a right to a fair hearing.   At the time you receive notice of service denial, termination or reduction, you will receive information and a form to fill out and mail in to claim this right.   The form should be mailed to:
